Abstract
Background: Congestive Heart Failure (CHF) is a condition when the heart is no longer able to pump enough blood to meet the body's circulation needs for the metabolism of body tissues under certain conditions, while the filling pressure into the heart is still quite high. One of the nursing problems that occur in patients with CHF is a decrease in cardiac output, which is where the heart is unable to pump blood to meet the body's metabolic needs which can cause edema in the lower extremities if action is not taken immediately there will be a buildup of fluid in the lung tissue which causes edema in the body.
Objective: Gain experience in carrying out nursing care for patients with CHF, using the nursing process, namely assessment, nursing diagnosis, planning, implementation, evaluation and documentation.
Methods: Using descriptive method with case study results and using a nursing process approach. Data collected through observation, interviews, physical examination and study documentation.
Results: The diagnoses in this case were decreased cardiac output, activity intolerance, knowledge deficiency, self-care deficit and risk of infection.
Conclusion: Of the four diagnoses that emerged, one diagnosis of knowledge deficiency was resolved, namely with the indicator that the patient said he understood the signs of congestive heart failure complications, the patient was able to repeat the definition, causes and prevention of congestive heart failure, while the diagnosis was partially resolved, namely a decrease in cardiac output, namely with indicators there is still a little edema, activity intolerance, namely the patient's walking distance indicator still needs to be monitored so that shortness of breath does not appear, and the risk of infection is that the patient's indicator is still infusion, therefore the patient is not free from the signs and symptoms of the risk of infection.
